Our office will verify your insurance as a courtesy, this is not a guarantee of payment.  We encourage our patients to verify their benefits themselves with their insurance carrier.

When you schedule a New Patient appointment through the online Appointment Scheduler or over the phone, you will receive an email that will provide a link to the online intake forms.

You will be prompted to enter your email and create a password in order to access and complete the New Patient online intake forms.

NOTE:  If you do not receive the email, please call our office at (916)861-0028 so we can send another email.
